We're on the lookout for a compassionate Home Hospice Registered Nurse for our client, a leader in providing heartfelt care in Sacramento,CA. Your role? To offer comfort, dignity, and peace to patients in their final days, right where they feel most at home. It's a calling for those ready to be a beacon of support, manage care plans meticulously, and coordinate closely with families and healthcare professionals.

Here’s what you’ll be doing:


  • Provide direct patient care in a home setting, assessing and responding to their needs with empathy and expertise.
  • Develop and oversee comprehensive care plans in collaboration with patients, families, and the healthcare team.
  • Manage symptom control and provide palliative care to improve quality of life.
  • Educate patients and their families on the hospice philosophy, ensuring they are fully informed and supported.
  • Coordinate with doctors, therapists, and other healthcare professionals to deliver holistic care.

Must-haves:


  • Current RN license with a passion for hospice care.
  • Experience in hospice or home health nursing, demonstrating profound empathy and professionalism.
  • Excellent communication skills, both for complex care coordination and for providing support and education to families.
  • A reliable vehicle and the willingness to travel within the community to provide patient care.
