JOB SUMMARY


THE NURSE CASE MANAGER WILL BE RESPONSIBLE FOR SCHEDULING CAREGIVERS AND ENSURING EFFECTIVE CARE COORDINATION FOR A CASELOAD OF CLIENTELE.Full time.  *Performs on call responsibilities and provides on call services to clients and families as assigned Monday-Thursday 5PM – 8AM and Friday 5PM through Monday 8AM rotating weeks – 7 days on and 7 days off.*Uses the Nursing process to preform all client related phone triage, addressing client and care provider concerns, in a timely manner and makes potential resulting home visits.• Presents a professional Impression over the phone although they may be in their home or in their vehicle they would give the appearance of being in a professional office.• Presents a professional appearance and demeanor when visiting new and existing clients to ensure high rate of conversion from prospect to client. *Obtain Doctor’s Telephone Verbal Orders when needed and fax Verbal order to be signed and returned by the physician the next day.*Completion of Incident reports for all after hours incidents and provide instruction to Care Providers RN, LVN, HHA.*Perform DocuSign Service Agreements with clients by email.• Performs initial assessments in client’s place of residence or hospital room or rehabilitation facility room to determine care needs and develop appropriate plans of care for Clients and their families,

. • Ensures that all service agreements and intake documentation are uploaded in SharePoint immediately but no later than 24 hours before the first shift. *Complete the Medication Reconciliation, assures the medications the client accesses are in agreement with those on the MAR and those ordered by the physician.*Complete the PO POC Physician Ordered Plan of Care at Admission*Complete a summary of Care to the PCP at admission*Performs Post Hospital Visits in client’s place of residence or hospital or Rehabilitation Facility Room to determine care needs and develop appropriate plans of care for Clients and their families,

*Complete a summary of Care to fax to the PCP at the Post Hospital Visit.• Engage the client and family with Update/Revision in the plan on Post Hospital Visits and as necessary to ensure quality and continuity of care. • Ensures the client’s Plan of Care is executed as written • Ensures reassessments of client’s care needs are performed when a patient has a health status change, upon the physician’s request, and after a hospital discharge. Complete a Summary of Care for the PCP and physicians actively participating in the clients care.• Ensures the care services team has all essential care plan information to include billing rates in the software to match care providers with clients. *Work as a team with Schedulers and Administrator on call to provide the most qualified and “best fit” Caregivers and Skilled Field Nurses for the client. • Onboards/ Teaches, and provides clinical direction to RNs, LPNs/LVNs and Caregivers providing them with the knowledge and skills to care for our complicated clients.• Provide on-site training and education to Care Providers as needed and ensure that caregivers are always introduced to clients on or before their first day in a clients’ residence.

Ensure that caregivers are educated on all relative signs and symptoms of a client’s diagnosis. • Promotes and educates the concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease. • Maximize sales potential by building a strong rapport with families, developing a thorough understanding of customer needs/requirements, and providing recommendations on the best arrangement of services (nursing, and/or care management).• Meets mandatory continuing education requirements of the company and licensing board.• Establish transparency with ongoing pricing and cost of services prior to completing the admission. • Ensure and comply with all required protocols and documentation for various payors (i.e., Long-Term Care Insurance and Workers Compensation). Assist the Client in providing documentation for obtaining the greatest number of service hours available to them for their care. • Ensures that the system is updated with appropriate documentation of all client communication (including all in-home follow-up visits and phone calls) and client records (e.g., plan of care) in a timely manner. • Obtains physicians orders as necessary for admissions, medication orders, treatments and wound care and ongoing clinical care.• Track various quality metrics including, but not limited to readmission rates for clients, diagnosis-related recovery trends, change in condition, hospitalizations, discharges, etc. • Uses infection control measures that protect both the staff and client (OSHA) • Perform other related duties or cross-cover as needed or as assigned. Requirements

QUALIFICATIONS AND EDUCATION REQUIREMENTS 


• RN required. • Strong candidates have a minimum of 1 year of experience working in the home care industry (or related industry – hospice, home health, hospital, etc.) *Strong Candidate will have experience with:the Care of Home Ventilator Patients, Tracheostomies, Cpap/bipap; Cough assist devices; Pleurx drains; Rocket drains, Nephrostomy drains; PICC lines, infusions for TPN, Dobutamine, Milrinone and Antibiotic; Ventricular Assist Devices; Traumatic Brain Injury and crushing Spinal Cord injuries with all levels or paralysis; transfer equipment Hoyer lift, Sit/Stand, e-pacer, ALS clients, Hospice and Pallative Care populations.They will have experience working with long term care insurance and Worker’s Compensation companies. 

  • High school diploma required.
  • At least 2 years of scheduling experience in home care required.
  • Experience in ClearCare preferred.

OTHER REQUIREMENTS 


• You must be able to lift up to 50 pounds at times with or without assistance. • Ability to drive when required throughout assigned region. • Prolonged periods of sitting at a desk and working on a computer.• Maintain current driving license and vehicle insurance. • Favorable background checkSalary Description$90,000 yearly
