Job Summary:Links Global is seeking compassionate and dedicated part-time Spanish-speaking Community Health Workers (CHW) to join our team and play a vital role in improving the health and well-being of our community in Montgomery County, Maryland. As a CHW, you will bridge the gap between healthcare and the community, serving as a trusted advocate, educator, and resource connector for our residents. Applicants should highlight their experience with in-person community outreach, public speaking, knowledge of community resources, facilitating community exercise activities (like walks), facilitating healthy eating workshops, organizing and coordinating community event activities, and navigating Spanish-speaking community groups in the county. A background check to meet government regulations is required.
Responsibilities:
- Build Trust and Relationships: Develop rapport with community members from diverse backgrounds to understand their health needs and concerns.
- Health Education and Promotion: Deliver educational workshops and individual sessions on various health topics, tailored to the community's needs (e.g., nutrition, chronic disease management, healthy behaviors).
- Resource Navigation: Connect individuals and families with essential health resources, such as medical care, social services, mental health services, and healthy food options.
- Case Management: Work with clients to develop personalized health plans, track progress, and provide ongoing support.
- Outreach and Screening: Conduct outreach activities to raise awareness about health issues and participate in health screenings (e.g., blood pressure checks, blood sugar screenings) and community health activities (walking, cooking, operating a booth at an event, and more)
- Advocacy: Identify and address systemic barriers affecting the community's health and advocate for improved access to healthcare services.
- Data Collection and Reporting: Maintain accurate client records and contribute to reports on community health needs and program effectiveness.
- Collaboration: Work collaboratively with healthcare providers, social workers, and other community partners to provide comprehensive care.
Qualifications:
- Two or more years of experience working in a community health setting.
- Fluency in Spanish and intermediate or above ability with English.
- High School diploma
- College degree is a plus
- Understanding of the community's culture, socioeconomic challenges, and health disparities.
- Excellent communication and interpersonal skills, with the ability to build rapport with people from diverse backgrounds.
- Outstanding interpersonal skills with a focus on active listening and empathy.
- Patience, cultural sensitivity, and a commitment to social justice.
- Strong organizational skills and the ability to manage multiple tasks effectively.
- Proficiency in basic computer applications (e.g., Microsoft Office Suite).
- Valid driver's license or reliable transportation is needed.
- Part-time availability.The rate of pay is $28 per hour. Local travel will be reimbursed per government regulations and allowances.
