JOB SUMMARY:


To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. To work as a team member to provide crisis intervention, support, safety plans and linkages to youth and families in Northeast Philadelphia, Region 1. The program operates 365 days/year from 8:00am – 10:00pm with rotating on-call coverage from 10:00pm – 8:00am.

QUALIFICATIONS: 


  • Bachelor’s degree in social work, counseling, marriage and family or psychology from an accredited college or university.
  • 2 years experience in the CASSP system.
  • Good interpersonal and communication skills.
  • COVID-19 Vaccination or willingness to receive vaccine required.
  • Active Driver's License required.

SPECIFIC DUTIES:


  • Provide crisis assessment, intervention , education and support in person, with assigned team member, within a designated time frame to referrals received by DBH/CBH.
  • Utilize approved evidence based practices and tools when providing crisis assessment and intervention, including SAMHSA crisis intervention guidelines
  • Assure all interventions are family focused and family driven.
  • Assist the family in the understanding of the events that led to the crisis from everyone’s perspective.
  • Assist the child/youth and family in obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation and education.
  • To provide follow-up care and/or transfer to other levels of care from the moment that an assessment is completed and the plan has been developed.
  • Assure appointments are scheduled at a time and place convenient for the family.
  • To complete or assist others in completing involuntary commitment petitions.
  • To arrange and follow through with voluntary or involuntary hospitalization admissions.
  • To follow all procedures regarding the initiation of a commitment.
  • Develop with the youth and family a safety/ crisis plan obtaining each individual’s perspective, strengths and ideas
  • Link the youth and family to the CRC or other 24/7 crisis staff and remain with the youth and family until the connection and collaboration is completed.
  • Provide rotating on call crisis coverage during the hours of 10:00pm – 8:00am to include phone crisis assessment, intervention and linkages. If in person contact is indicated, review with on call supervisor before any implementation.
  • Be knowledgeable of local community resources and services and provide care coordination.
  • Provide up to 72 hours of intervention by scheduling follow up appointments at a time convenient for the family, which includes evening and weekend appointments in the home and community.
  • Provide targeted one to one support of identified youth as directed.
  • Other duties as assigned.

Our roots can be traced back to 1973 when PATH was incorporated as a not-for-profit corporation. This came about from the efforts of a group of dedicated community members, many of whom had family members in need of services, which were scarce to non-existent at the time. They worked tirelessly with a group of caring professionals to form PATH and to secure funding in order to provide much needed services to the citizens of Northeast Philadelphia. This tradition of the involvement of dedicated and compassionate community members continues today through our volunteer Board of Directors who are all residents of Northeast Philadelphia. The Board of Directors, which is largely composed of family members of individuals who participate in our programs, continues to strongly advocate for the needs of the community. Over the years, our community has expanded to include individuals throughout the city of Philadelphia and in surrounding counties through our Specialized Russian services Starting with a staff of 14 in rented office space providing only information and referral services, PATH has significantly expanded its programs as we currently serve over 3,000 individuals annually in our various Behavioral Health Programs, which include both Mental Health and Substance Use Treatment, as well as in our Intellectual Disabilities programs. With over 500 competent and considerate staff members, we are able to provide a wide array of quality services to meet individuals' needs. Our facilities include our main campus at 8220 Castor Avenue, 4 satellite locations, and over 70 homes in the community where we provide residential supports to enable individuals to remain living in the community. We also provide school based services in numerous Philadelphia schools. We have remained true to our mission of helping individuals achieve a more independent and fulfilling life.
