Overview
The Registered Nurse works in collaboration with the multidisciplinary team to improve the health of clients through health education, medication administration and care coordination. Key Role Responsibilities
Perform clinical tasks including client education, medication administration, vaccine administration, medication adherence, review of preventive health needs and nursing assessments.
Accurately assess client’s needs and coordinates access to appropriate services for clients and assists with navigating the health care setting.
Deliver health education and counseling, drawing upon the individual’s strengths, resiliency and motivation. This may include delivering interventions that demonstrate an appreciation for the impact of trauma and, when warranted, utilizing clinical approaches such as harm reduction and motivational interviewing, etc. to support the individual’s wellbeing, improve health care utilization, and reduce risk behaviors.
Actively participate on an integrated, multidisciplinary team to develop and implement an integrated treatment plan, within the context of an individual or family’s unique social needs, as well as contribute to interdisciplinary case conferences on an ongoing basis
Accurately complete documentation of services in the electronic medical record in a timely manner
Use good judgment in the use of one’s time and the time of others
Actively seeks information from appropriate sources and incorporates evidenced based guidelines in clinical decision making.

Knowledge, Experience, and Skills Formal Education and Training
Possess current licensure as a Registered Nurse in Maryland
BSN required
BLS CPR certification
Experience
Two years of clinical experience as a Registered Nurse
Experience in community health or primary care setting as an RN is preferred
Comfortable working with individuals and families experiencing homelessness and poverty
Skills
Strong interpersonal skills necessary to work with vulnerable populations and to collaborate with other medical team members, other staff members, and personnel of other agencies
Possess ability to make independent decisions when circumstances warrant such action
Able to cope with interruptions, be flexible and be a team player
Able to work with ill, disabled, emotionally upset and sometimes hostile individuals
