Overview
The Pediatric and Family Case Manager provides office and field-based case management services to increase access to health care and social services for clients. The position works on a multi-disciplinary care team, as well as with other case managers and community partners to promote client and family wellness across the agency.Key Role Responsibilities
- Provides case management assessments, treatment planning and interventions. Engages clients and families and builds relationships by greeting clients with a warm introduction, communicating with empathy and compassion, and working on goals identified by the client and their family.Collaborates with clients in the development and implementation of an integrated treatment plan using SMART goals.
- Systematically identifies, documents and shares resources that benefit clients and their families. Reviews and approves financial assistance requests according to established procedures. Assertively supports connection to financial assistance, insurance, entitlements, services for immigrants and referrals for other services to remove barriers to care.
- Collaborates with agency providers in an integrated, multidisciplinary care team, attending interdisciplinary meetings as required. Engages team members in problem solving regarding client and family care, and links clients and families to ongoing medical, behavioral health and supportive services.
- Develops own racial equity and inclusion lens and commits to continuous self-education to improve the agency’s provision of equitable and inclusive care. Works to identify and reduce health disparities of clients at population and individual level. Uses a population health approach to proactively identify clients and families who need interventions in the areas of income, housing and other social determinants of health and for those who may need reconnection to care.
- Provides subject matter expertise in case management for youth and families, with a focus on those without medical insurance, including immigrant populations.
- Nurtures and develops community partnerships to increase youth and family access to medical and behavioral health care as well as developmental, parenting and social services. Supports access for people who are not yet connect to care at the agency.
- Completes documentation of services in a legible manner in accordance with established formats and required timeframes within the client electronic health record.

- Performs other duties on an as-needed basis
- Protects clients’ personal health information by maintaining compliance with HIPAA and other relevant health care related IT security regulations
Knowledge, Experience and Skills
Formal Education and Training
- Bachelor’s Degree in human services field preferred: five years of relevant work experience may be substituted.
- Personal vehicle and valid Maryland driver’s license required
- Bilingual Spanish-English highly preferred (premium pay offered)
Experience
- Three years of experience providing case management services to vulnerable populations preferred
- One year of experience in a health care setting or community-based organization required
- Experience working with individuals from diverse backgrounds required
Skills
- Knowledge of assessment and treatment planning
- Able to describe how they support and uplift BIPOC and TGNC individuals and communities in their work
- Well-developed interpersonal skills necessary to engage clients and promote positive relationships with other community agencies and providers
- Able to describe how they support and uplift BIPOC (Black, Indigenous and People of Color) and TGNC (transgender and gender-nonconrming) individuals and communities in their work
- Willingness to integrate principles into practice such as Harm Reduction, Motivational Interviewing and Housing First
- Strong verbal and written communication skills
- Strong organizational and time management skills
- Able to cope with interruptions and be a team player
- Flexible approach, working with several cross-disciplinary teams in a collaborative style
- Approaches change with a positive, open-minded attitude
- Able to work with clients who have significant mental health challenges, manage crisis situations and assess for safety
- Demonstrated understanding of the psychosocial issues faced by underserved populations, including homeless individuals
