Job Summary
The IOP & Adolescent Program School Liaison is responsible for coordinating care needs for IOP/Adolescent program clients. This employee is responsible for oversight, administration, and direct service of all functions between the IOP/Adolescent Program and participating schools. This includes but is not limited to, outreach for engagement of new referrals from schools, school transition meetings, coordinating absences from school, being available to be a representative in the school, and knowledge and understanding of clinical work and presentation of clients participating in the program.
Essential Responsibilities and Metrics
- Facilitate client referrals for services with assessed needs, school recommendations, and desired outcomes
- Provide appropriate linkage, referral, coordination, and follow-up for clients to services and supports
- Regularly consult with treatment team and school professionals
- Maintain documentation in a timely manner, including, but not limited to, tracking school engagement, client attendance to account for absences, school transition meetings
- Represent the agency with integrity and professionalism
- Participate in community-related services as appropriate
Qualifications
- Bachelor’s degree in human services field.
- Demonstrate competence in interdisciplinary and inter-agency collaboration
- Minimum of 1 year relevant experience working with individuals experiencing mental illness
- Experience within school system and developing relationships with school professionals
- Advanced oral and written communication skills
- Ability to think critically and analytically and to problem solve in both short and long term contexts
