- Has 7 health center sites in underserved communities throughout Prince George’s and Montgomery Counties serving over 34,000 patients.
- Oversees several federal, state, and county contracts and/or grants that focus on clinical operations, patient outcomes, and access to care and services.
- Provides health assessments for refugees, asylees, and special visa holders for the state of Maryland. The program currently sees around 2,100 patients on an annual basis.
POSITION SUMMARY:The Telehealth Triage Registered Nurse (Triage RN) under the direction of the Lead RN – Triage and Director of Nursing, along with the clinical care team, serves as an integral member of the nursing leadership team. The Triage RN assesses patient needs, prioritizes according to urgency, provides evidence-based nursing advice and initiates or directs care to the appropriate resources over the phone and through the electronic medical record (EMR) for CCI’s patient population.
Key Functions and Responsibilities
- Utilizes evidence-based resources to provide telehealth nursing triage services to patients across the lifespan: assesses medical history, clinical need and urgency and determines necessary disposition via telephone or web consult.
- Provides patient education, medical advice and preventative behaviors that support the patient’s signs and symptoms.
- Reaches patients for transition of care and ensures follow up for patients following hospital discharge or emergency room visit.
- Engages in CCI quality assurance and quality improvement activities.
- Attends and participates in meetings, committees, health fairs, events and trainings as assigned.
- Consistently seeks avenues for self-education to maintain compliance with evidence-based practice and advancements in triage nursing.
- Maintains the patient’s electronic medical record within expected timeframes, following CCI standards for charting.
- Provides quality customer service and communicates in a HIPAA compliant, culturally sensitive and trauma-informed manner.
- Works within the policies and procedures of CCI Health Services Inc.
- Performs other duties responsibilities as assigned by the Lead RN – Triage and the Director of Nursing or designee.
Minimum Qualifications:
- Active RN license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact.
- Minimum of two (2) years of nursing experience, with acute care preferred.
- Current BLS/CPR certification required.
- Excellent written, verbal, telephone, and interpersonal communication skills with experience documenting in an Electronic Medical Record (EMR). Specific knowledge of eClinicalWorks (eCW) is a plus.
Written and oral fluency in English. Bilingual (English/Spanish) preferred.
