To provide direct consultation and support services on behalf of Synapticure to the neurodegenerative community across the US. You will be in charge of providing care coordination and support services to patients, caregivers and loved ones affected by neurodegenerative disease, such as ALS, Parkinson’s Dementia and Huntington’s, and building relationships with partners to drive real, measurable change in how people affected by neurodegenerative diseases are treated.
Synapticure is a for-profit entity that aims to give all patients living with neurodegenerative disease access to the best care while fueling the advancement of personalized treatments. We do so by creating a guided journey for each patient, their caregiver and family that educates and empowers while facilitating access to critical tests and leading experts. De-identified data is then used to both inform personalized treatments for patients today and to speed the development of new targeted therapies.Synapticure launched in early 2022, initially serving individuals living with ALS and has now expanded to serve additional neurodegenerative diseases.

Responsibilities
The Behavioral Care Manager will be responsible for supporting members of the neurodegenerative community and our partners through phone, email, in person, web portals, chat and texting communications. In collaboration with and under the guidance of the Director of Behavioral Health Services and other team members, you will execute on the Synapticure collaborative care program, ensuring the mental health needs of this population are met.
Role
The Behavioral Care Manager is a direct support mental health needs to people affected by neurodegenerative diseases. This person oversees the relationship, enrollment, documentation and care experience, and requirements of the collaborative care model.
Essential Functions
- Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers.
- Screen and assess patients for common mental health and substance abuse disorders.
- Facilitate patient engagement and follow-up care.
- Provide patient education about common mental health and substance abuse disorders and the available treatment options.
- Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
- Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
- Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
- Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.
- Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
- Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
- Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultants, and other treating providers.
- Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care. - Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
- Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Minimum Qualifications
- Bachelor's Degree in a related field
Preferred Qualifications
- RN or LCSW licensure
- 2+ years' experience in direct case or care management.
- Experience in ALS, Parkinson's Disease, neurodegenerative, chronic or rare disease space.
- Exceptional communication skills.
- Experience with insurance and billing/coding.
- Experience handling and reviewing medical records.
- Experience with clinical trials.
- Experience in a startup or organization that has experienced rapid growth and change.
Travel Requirements:
This position is remote and work would be based in your home. The necessary technology would be provided to work out of your home. Travel to Chicago, IL where headquarters are and/or other meetings may be expected.
