Position Summary


The Medical Assistant Care Coordinator works under the direction of the Southwest Center's healthcare providers and reports directly to the Clinic Operations Manager.  In this position, you will play a pivotal role in comprehensive care coordination.  This includes collaborating with interdisciplinary teams to develop personalized care plans, managing referrals and prior authorizations, and ensuring that patients receive the appropriate resources and support throughout their healthcare journey. In this position, you will be responsible for assisting support staff and providers in establishing and coordinating care services, you will be ensuring seamless transitions between different levels of care, and you will be facilitating effective communication among all stakeholders.

You will be fostering strong patient-provider relationships and ensuring appropriate follow-up care.  This role is vital in improving patient outcomes and enhancing the overall patient experience within our care.As a Medical Assistant Care Coordinator, you will support the Southwest Center's mission of providing affirming and inclusive services to promote well-being and advance health equity for diverse communities and all those seeking compassionate care - especially people of color, LGBTQIA2S+ and Queer individuals, and those affected by HIV.  

What your day will look like at Southwest Center


Responsibilities include, but are not limited to:

  • You will prepare for patient appointments by reviewing and updating patient records prior to patient arrival.
  • You will coordinate appointments for specific care and diagnostic procedures.
  • You will report communicable diseases to agencies as required.
  • You will initiate and manage prior authorizations for medications and procedures.
  • You will be responsible for the tracking of laboratory and diagnostic tests.
  • You will review patient gaps in care reports to ensure appropriate follow-up of required patient care.  In addition, you will be responsible for contacting and scheduling of patients identified.
  • You will complete next day follow-up visit calls to patients.
  • You will assess challenges, situations, or issues presented and appropriately prioritize while assisting with resolution using strong critical thinking and problem-solving skills.
  • You may participate in orientation, mentoring, and training of new and established staff.
  • You will be working with various healthcare and support staff to assure smooth and efficient patient flow and clinical operations including patient scheduling, inventory maintenance, prescription and lab processing, lab draws, assisting with procedures, and preparing/administering medications and injections via multiple routes for various treatment purposes. 
  • You will foster client focused care such as obtaining, communicating, and recording patient related information including vital signs, drug allergies, medications, medical histories, and reason for visit in Electronic Health Records (EHR, EClinicalWorks) in a timely manner. 
  • You will provide HIV and STI education services to individuals at high-risk for HIV infection. You may facilitate and process blood (finger stick and venipuncture) specimens and prepare specimens for laboratory processing. You may also provide HIV test results to clients. 
  • You will perform miscellaneous job-related duties as assigned including, but not limited to, processing referrals, medical records reconciliation, completing prior authorizations, triaging phone calls, and scanning/uploading documents into patient charts. 
  • You will also cross-train in front office duties to ensure coverage in the event operations requires them to float to assist with coverage.

Requirements

Who we're looking for:


  • You have 2 or more years of experience as a Medical Assistant
  • You've earned a high school diploma or completed your GED.
  • You have graduated and received a certificate of completion or diploma from an approved and accredited (CAAHEP or ABHES) medical assisting training program
  • You have demonstrated cultural competency to professionally and respectfully serve the LGBTQIAS+ communities
  • You must be professional, flexible, caring, and compassionate, while exhibiting a congenial and sensitive attitude toward patients
  • You have excellent computer skills with proficiency in Microsoft Office (Word, Excel, Power Point, Outlook) and EHR software
  • You are a highly organized, detail-oriented self-starter, that possess excellent problem-solving skills.  You also must have the ability to assess organizational needs, you can establish priorities and multi-task effectively while maintaining a positive attitude
  • You possess strong interpersonal skills in dealing with a variety of people and proven ability to work independently while collaborating with others

  • You have experience with Patient Centered Medical Home models of care
  • You have experience in Integrated Care settings
  • You have experience working in eClinicalWorks
  • You are bilingual in Spanish and English

Environmental Factors, Physical Requirements, and Compliance


  • Work primarily in a climate-controlled environment with minimal safety/health hazard potential
  • Must be able to work at least two evenings per week
  • Office environment, exposure to computer screens for lengthy periods of time
  • Travel outside of the Phoenix region approximately 10% of the time
  • Currently have, or be able to obtain within 90 days of employment, a valid Fingerprint Clearance Card
  • Currently have, or are able to obtain within 7 days of employment, a clear TB test
  • Currently have, or are able to obtain within 7 days of employment, a current flu vaccination
  • Currently have, or are able to initiate within 7 days of employment, a Hepatitis-B primary vaccination series
  • Currently have, or are able to obtain within 14 days of employment, a CPR certification
