As a Clinical Programs Coordinator, you'll support the administrative functions of the Prevention and Navigation departments to enhance patient engagement and retention. In this role you'll use your exceptional attention to detail for data entry and documentation tasks. Using your high level of cultural competency, you'll to address the unique needs of transgender individuals and the broader LGBTQIA2S+ community. While there is minimal face-to-face interaction with patients, as a Clinical Programs Coordinator you'll emphasize customer service and virtual engagement to manage your caseload. The Clinical Programs Coordinator supports the Southwest Center mission of providing affirming and inclusive services to promote well-being and advance health equity for diverse communities and all those seeking compassionate care - especially people of color, LGBTQIA2S+ and Queer individuals, and those affected by HIV.
What your day will look like at Southwest Center:
- You'll work collaboratively with the Clinical Programs Department and Wellness Clinic to support administrative functions, such as documenting encounters, lab results, communicable diseases, and linkage to care into appropriate internal, state, and CDC databases.
- You'll showcase exceptional customer and patient experience while contacting individuals to provide testing and lab results, as well as engaging with individuals who have fallen out of care or are at risk of falling out of care within the Wellness Clinic.
- You'll provide support with indirect access to testing services through the preparation of specimens for no-cost testing, including handling bodily fluids and ensuring proper charting has been completed for each encounter.
- You'll directly collaborate with local health departments regarding communicable diseases and infection control as it relates to the Clinical Programs Department, in addition to ensuring patients are successfully linked to care.
- You'll complete the daily transportation of testing specimens and lab results to and from local health departments and laboratories.
- You'll provide information and referrals for other partnering agencies that provide additional support and services that cannot be met internally.
- You'll provide clear, understandable, current, and accurate information to clients, community members, and volunteers. You'll provide prevention and harm reduction information in a way that shows respect, listening skills, empathy, and patience in a non-judgmental, stigma-free way.
- You'll work collaboratively with other internal Clinical Programs to further develop and evolve the needs of the department, including but not limited to, policy reformation, training new-hires, and identifying opportunities for growth and improvement within the agency.

Requirements
Who we're looking for:
- You're a high school graduate, or you've earned your GED.
- You're proficient in Microsoft Office (Word, Excel, Outlook, PowerPoint) software applications and competent in using electronic medical records to complete effective documentation of patient care.
- You have a full understanding of HIPAA and protecting patient confidentiality.
- You've demonstrated a sound understanding of gender identity, gender expression, gender presentation, sex, and sexual orientation etc. This includes a strong grasp of local and national resources (medical, mental health, social etc.) for those that are transgender or gender non-conforming.
- You possess a valid driver’s license and personal vehicle to conduct programmatic business.
- You're a highly organized self-starter with excellent problem-solving skills, ability to multi-task effectively, and a willingness to learn.
- You've demonstrated excellent written and verbal communication.
- You have the ability to work in fast paced environment with changing deadlines and priorities while maintaining a positive attitude.
- You've demonstrated cultural competency to professionally and respectfully serve the LGBTQIA2S+ communities.

- You have personal or professional experience regarding members of the transgender and/or non-binary community, as well as all components of the LGBTQIA2S+ community.
- You have experience using Electronic Medical Records.
- You're bilingual in Spanish and English.
Environmental Factors, Physical Requirements, and Compliance
- Work primarily in a climate-controlled environment with minimal safety/health hazard potential.
- Office environment, exposure to computer screens for lengthy periods of time.
- This is an exempt position that may require evening and occasional weekend work.
- Travel outside of the Phoenix region approximately 10% of the time.
- While performing the duties of this job, the employee is regularly required to walk, sit, stand; use hands, talk, and hear. The employee is occasionally required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus.
- Currently have, or be able to obtain within 90 days of employment, a valid Fingerprint Clearance Card
- Currently have, or are able to obtain within 7 days of employment, a clear TB test
- Currently have, or are able to obtain within 7 days of employment, a current flu vaccination
- Currently have, or are able to initiate within 7 days of employment, a Hepatitis-B primary vaccination series
- Currently have, or are able to obtain within 14 days of employment, a CPR certification
