The Role
Working in small teams with other highly capable and passionate care team members , the Registered Nurse (RN) will play a key role in completing comprehensive evaluations and administering holistic assessments to support the diagnosis, and treatment of patients impacted by dementia. This role involves collaborating with an interdisciplinary team and Rippl Medical Director to develop and implement individualized dementia specific treatment plans that address patient and caregiver needs and promote overall well-being.
This individual thrives in an innovative environment, appreciates working collaboratively in interdisciplinary teams and desires to improve the health care experience for persons with dementia and their caregivers. This role demands a detail oriented clinician who strives to develop new dementia care solutions and drive process improvement to elevate clinical quality.
Responsibilities:
- Develop high-quality therapeutic alliances with patients and their caregivers by providing direct patient care for dementia issues
- In-depth medical record review and summarization to support dementia comprehensive evaluations
- Administer dementia specific assessments to patients and caregivers such as: Functional Assessment Staging Tool (FAST), Zarit Burden Index (ZBI), Health Related Social Need (HRSN).
- Complete medication reconciliation with emphasis on identification of high risk medications in dementia
- Work closely with Rippl ARNPs and Dementia Physicians to evaluate, diagnosis, stage and develop treatment plans for dementia patients
- Evaluate caregiver stress and strain through a validated assessment and support care plan to improve
- Identify process improvement and quality enhancements in clinical pathways
- Identifies opportunities to improve the patient and caregiver experience
- Guide patients and their families through the journey of dementia
- Supports interdisciplinary team meetings, case conferences, and consultations to discuss patient care and treatment strategies
- Coordinate patient care with primary care providers and specialists, nurses and home health aides, including handling more complex cases
- Assist in advanced care planning for both patients and families
- Stay informed about current research, best practices, and evidence-based treatments in dementia care through continuing education and professional development activities
- Adhere to legal and ethical standards of practice, including maintaining patient confidentiality and upholding professional boundaries
Qualifications:
- An unrestricted, current compact Registered Nurse (RN) license in WA, TX or IL and residency within a compact-eligible state required
- Geriatric Medicine, Palliative Care or Chronic Care management experience preferred
- Unrestricted ability & willingness to license in other 50 states per business needs
- A deep passion for taking care of older patients and developing longitudinal care relationships with them and their families
- Interest in learning a new skill set and pioneering a new workforce
- Comfort and/or experience working with interdisciplinary teams
- 1-2+ years of experience managing older adult patients in outpatient, inpatient or long-term care settings (preferred but not required)
- Strong clinical assessment and diagnostic skills, with the ability to formulate comprehensive treatment plans
- Excellent communication and interpersonal skills, with the ability to establish rapport and build therapeutic relationships with patients and their families
- Proficiency in electronic health record (EHR) systems and other healthcare technology tools (Athena preferred)
- Ability to work collaboratively as part of a multidisciplinary team and to function independently with minimal supervision
- Preferred experience with telehealth
- Commitment to professional growth and ongoing learning in the field of geriatric nursing
- Fully remote role with limited weekend or after hours work
- Ability to work 4 - 10 hour days

Pay Range Details
The pay range(s) below are provided in compliance with state specific laws.
