In fact, Americans living in rural communities suffer a mortality rate 23 percent higher than those in urban communities, in part because of the lack of access to quality care.

The Opportunity


Join us in tackling healthcare for rural America! We’re seeking a
Market Innovation Manager who is passionate about having positive societal impact and using technology for good—solving issues of health inequity for communities that have been perennially underserved.You will design, develop, and deploy innovative solutions that improve contract performance and efficiency.  You will work closely with internal and external stakeholders to scope out and manage short and intermediate-term projects. You will be accountable to market leadership for driving your projects to completion, and will have many high visibility opportunities to demonstrate your impact to Homeward leadership.

  You are a keystone role to marshaling all of Homeward’s resources to enhance patients’ care.

What You'll Do:


  • Support relationship management with payer partners, through leading short and intermediate projects in collaboration with key payer stakeholders and in support of driving performance on market objectives. 
  • Support relationship management with provider partners, including relationship and performance management, for both contracted and non-contracted providers in the market. 
  • Work cross-functionally to design and administer internal projects, with the goal of enhancing Homeward's value proposition to patients, providers, and payers, or to improve Homeward's ability to measure its performance. 
  • Work with in-market stakeholders, such as Practice Management, Provider Outreach, and Medical Directors, to evaluate the success of initiatives.
  • Work closely with Market President to design, scope out, and deploy new initiatives. 

What You Bring:


  • 7-10 years of hands-on operations experience with value-based care, healthcare delivery, data analysis, and project management.
  • Expert level proficiency in Microsoft Office, especially Excel and PowerPoint. At least 2-3 years experience designing and delivering executive presentations.
  • Content expertise related to Medicare Advantage outcomes, such as HEDIS, Star Ratings, Risk Adjustment, and Affordability.
  • Superior track record of compliance leadership, including audit experience and expert knowledge of federal and state regulations.
  • Experience with provider engagement across internal, affiliated, and other types of provider partners.
  • Stellar cross-functional collaboration with a variety of stakeholders.

The base salary range for this position is $90,000 - $100,000 annually. Compensation may vary outside of this range depending on a number of factors, including a candidate’s qualifications, skills, location, competencies and experience. Base pay is one part of the Total Package that is provided to compensate and recognize employees for their work at Homeward Health.
