The Opportunity


We seek a
full-time Administrative Care Navigator (Medical Assistant, Licensed Practical Nurse) passionate about helping people in rural communities throughout Stearns county.You’ll be responsible for working with Homeward members and their primary care physicians to achieve their best health, while also providing support to the Clinical Navigators in your designated geography. Expectations within this role include member chart preparation, completing historical reviews to support clinical documentation, assisting in obtaining and processing medical records, mailing visit summaries, and conducting telephonic outreach to build relationships with members, assisting to connect them with services they need, both clinical and beyond, and address gaps in their care.  You will deeply understand the local community and use your expertise to advance members’ health.Homeward offers unique career opportunities that underscore our vision and values, recognizing what matters most to our clinical team – time with their patients.

What You'll Do


  • Engage potential members by effectively communicating the services and value that Homeward can provide
  • Initiate communication with members on completing pre-appointment requirements (e.g., registration forms, lab tests, x-rays, etc.)
  • Initiate encounters with members to prepare for the visit, coordinate member flow, gather and document pertinent data (vitals, medications, allergies) from the member, and enter information into the medical record
  • Ensure accurate member records by obtaining release of information, reviewing medical history, medications, allergies, and treatments administered
  • Building and maintaining the members' internal medical record for clinical engagement
  • Conduct check-ins and/or visits with members telephonically
  • Facilitate the processing of prescriptions by performing a complete review of medications with the member and updating the medication list with changes as appropriate
  • Schedule necessary appointments for clinical and navigation needs
  • Collaborate with members’ primary care providers and their teams to promote cohesive care
  • Build for scale by identifying and maintaining a list of community resources and contacts to meet our members’ needs
  • Support fellow Navigators in maintaining member panels within the designated geography

What You Bring


  • Completion of a Medical Assistant or LPN program from an accredited program or school 
  • High school diploma or equivalent
  • Licensed/eligible to practice in the United States
  • At least two years of experience in high-touch, patient-facing roles, preferably with seniors
  • Passion for delivering care in rural America and ability to persuade members to take actions that support their health
  • Demonstrated proficiency with EHRs and other technology that supports outreach (Epic, Cerner, Athena, etc.)
  • Expertise with the local community, geography, culture, healthcare ecosystem, and available resources

Bonus Points:


  • Previous experience with care navigation
  • Previous experience in a fast-paced, high-growth startup environment
  • Experience working with connected devices/loT is a plus
  • Experience with medical or health data is a plus

The base salary range for this position is $23-28/hour. Base pay is one part of the Total Package that is provided to compensate and recognize employees for their work at Homeward Health.
