We have an exciting opportunity for a
Long-Term Services and Support Specialist.Altarum’s Delivery System Transformation (DST) Practice Area is seeking a Long-Term Services and Supports (LTSS) Specialist with direct experience in nursing homes or other residential care organizations (e.g., assisted living facilities, home health care, etc.,). The DST team has deep roots in enhancing policy design, understanding and improving implementation, and leveraging policy to help create a broad public commitment to high-quality, accessible care for older adults and people with disabilities.  The successful candidate will provide a range of support through liaising with participating nursing homes partners to support and guide them through culture change efforts, coaching partners around barriers to implementing and sustaining culture change within their community and, engaging them in robust quantitative and qualitative data collection to inform their continued implementation efforts.

Occasional travel (quarterly) to visit nursing home partners is required.  This position requires working effectively with diverse team members, management, and consultants. Responsibilities


  • Provide support for continued implementation and enhancement of Altarum’s comprehensive culture change projects. 
  • Act as a liaison for partnered nursing homes: maintaining collaboration and engagement with them. Including assistance with monthly data submissions, creatively evaluate and eliminate obstacles to data tracking and submission, and craft resources and materials to support nursing homes in their culture change efforts. Previous training experience is helpful. 
  • Lead qualitative interviews and focus groups with nursing home residents, staff and families and distill qualitative data for publications. 
  • Consolidate quantitative and qualitative data to inform partners' progress and identify areas to concentrate efforts to ensure forward momentum continues. 
  • Identify and mitigate barriers that are inhibiting growth.  
  • Monitor individual nursing home outcomes and ensure partners and project team members are well informed of their status and progress. 
  • Assist in crafting publications and presentations that communicate evaluation findings to a range of audiences, including project partners, funders, advocates, and other key stakeholders. 
  • Leverage Quality Assurance Performance Improvement principles to build sustainable models of culture change in partnered nursing homes. 
  • Support development of reports, case studies, site visit summaries and other writing, as required  
  • Provide additional project support as needed. 

Qualifications


  • Bachelor’s degree and/or Master’s degree in a related field is preferred with at least 6 years of professional experience in health services or a related field  
  • Strong interest in health care policy, long term services and supports, and health care service delivery 
  • Knowledge of quality improvement frameworks and processes preferred, but not required. 
  • Direct experience in residential long-term care settings preferred, but not required.  
  • Demonstrated experience in population health-based research and evaluation. 
  • Ability to synthesize data and research concisely in easy to access, plain language, materials for nursing home partners, residents, engaged caregivers, state leaders, and other audiences.  
  • Desire to integrate research, technical, analytical, and consulting skills to serve the public good and improve human health 

Logistical Requirements


  • At this time we will only accept candidates who are presently eligible to work in the United States and will not require sponsorship 
  • Our organization requires that all work, for the duration of your employment, must be completed in the continental U.S. unless required by contract 
  • Must be able to work during eastern time unless approved by your manager
