JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Work primarily with children and adolescents with serious mental illness or emotional disorders and their families, with the goal to maintain the youth in the least restrictive setting with regards to home, school and community programs. Work closely with youth and their families in providing the support, training and assistance required for a stable and healthy family and community life.
QUALIFICATIONS:
- Bachelor’s Degree in Human Services field from an accredited college/university
- One to two years experience in the behavioral health field
- Use of own personal vehicle and valid driver’s license required
- Experience working with children and adolescents preferred
- Ability to use electronic health record required
SPECIFIC DUTIES:
Join the intervention specialist in the initial meetings with an assigned youth and family to begin engagement process and identification of resources to enhance treatment and/or improve social detriments of health.Assist the family in understanding of the events that led to the crisis from everyone’s perspective.Assist the intervention specialist in provision of crisis intervention, management, parent education and treatment interventions.Develop with each child/youth and family, a unified service plan addressing the needs for treatment, skill development, social and emotional development and social service needs Coordinate psychiatric services as indicated for assigned children/youth with the CMIS psychiatrist or other provider already involvedAssist the child/youth and family in obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation, education.Provide home, community and school visits to each assigned child/youth and family as assigned and indicated.
