Your Mission:
- Deliver exceptional primary care to a dedicated panel of patients, focusing on their unique health goals.
- Champion the 'treat in place' model, ensuring advanced care planning is seamless and tailored to each individual.

What You'll Bring:
- Unrestricted state licensure as an Advanced Practice Nurse and national board certification.
- Medicare Registered.
- DEA License
- 6+ months of experience as an Advanced Practice Provider (preferably with geriatric, hospice, or palliative care populations).
- Strong clinical skills and experience in treating chronically ill patients, with a preference for nursing home environments.
- Tech-savvy with excellent communication skills to collaborate effectively with patients and healthcare teams.
