Our medication management programs and processes are proven to improve outcomes while reducing overall healthcare costs for our patients.Curant Health is searching for a Nurse Care Manager to join its team in Smyrna, GA.As the Nurse Care Manager, we will look to you to perform care management duties to assess and assist patients and their support systems in managing chronic medical conditions more effectively to promote quality, cost effective care.
Responsibilities

We have listed a few of your responsibilities below:
- Assess the patient’s current health status, resource utilization, past and present treatment plan and services, and short and long-term goals.
- Assess and adjust the care plan and promote desired outcome.
- Provide patient and provider education.
- Enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems.
- Coordinate with patient’s Primary Care Physician (PCP), specialists, medical providers, and non-medical staff as necessary to meet the needs of the patient (and clients).
- Provide proper follow-up and monitoring with patients and prescribers/healthcare providers.
Qualifications
- Bachelor’s degree in Nursing preferred
- 2+ years of clinical nursing experience in a clinical, acute care, or community setting
- Demonstrated experience working with patients to coordinate and improve care.
- LPN or RN License required
