Summary
The Supervisor of Case Management is responsible for the leadership and functioning of the Case Management Team serving single women and men living with HIV/AIDS while experiencing homelessness. Oversees case managers and drives performance to support care delivery with the purpose of helping clients obtain stable income and housing with an emphasis on maintaining health outcomes. Adheres to budgets, monitors program goals and evaluations, delegates responsibilities, and assists the manager with maintaining policies and procedures.
Represents the program and organization both on-site and throughout the community.
Essential Functions
- Recruits, recommends candidates for hire, and onboards the Case Managers I, II, the Community Social Worker, and Interns in the team to implement best practices in Social Services and Case Management
- Facilitates/conducts trainings; assigns tasks and monitors work performance; writes and delivers performance appraisals; recommends salary actions; recommends and administers disciplinary actions; recommends termination of employees and engages and retains a high-performing, accountable workforce.
- Manages a caseload of clients, providing case management services as needed.
- Monitors and evaluates staff work to include, caseload assignments, client interaction, interventions, referrals, and timely documentation of services.
- Plans, coordinates, and facilitates client orientation, meetings, interventions, and events.
- Creates and communicates work schedules and staff meetings to the department and other programs; prepares, participates/presents at MAS (multidisciplinary approach to services) meetings.
- Maintains grant compliance, and provides data/updates as requested.
- Maintains the organization’s CSTAR client and city’s (Clarity) databases, pulls and reviews monthly reports, and discusses progress and needs with the staff. Performs weekly file reviews and delivers the review results to the staff.
- Updates goals in ADP quarterly throughout the year. Tracks expenses for the program and communicates discrepancies to the Manager of Case Management.
- Utilizes a strength-based / trauma-informed approach to services, uses Motivational Interviewing techniques, and CREED to encourage positive change.
- On-time completion of assigned training and policies.
- Performs other duties as assigned.
Qualifications
- Bachelor’ degree in social services (social work, psychology, sociology, etc.), or equivalent experience; and
- Two (2) years of experience Supervising in a high-demand environment, and
- Two (2) years of experience in case management, and
- Four (4) years of experience working with underserved populations in a social services setting
- Must - understanding of HIV/AIDS and related medical information
- Must - understanding of Substance Use Disorders
- Bilingual (English and Spanish) is a plus.
- Demonstrated basic understanding of principles of Housing First, Trauma Informed Care, Conflict Resolution, Motivational Interviewing, Low Barrier Operations and Prevention and Diversion practices, social services, case management, eviction prevention, and crisis intervention.
- Basic user of MS Office.
- Ability to pass fingerprinting and background checks upon employment.
- Must have a valid California Driver’s License and be able to qualify for insurance coverage.
