Summary:
Lead OBRA Therapist oversees the staff assigned to provide behavioral health services to nursing home residents, including Therapy and Social Work. They are responsible for the day-to-day operations of the OBRA Team, ensuring nursing h9ome residents receive adequate behavioral health services, in accordance with state guidelines. This is a clinical and administrative position which involves the orientation and training of new staff, reviewing and assuring accuracy of documentation, case assignments, collaborate with nursing homes and hospitals to facilitate effective provision of OBRA/PASRR services, consultation and education regarding OBRA Specialized Services pursuant to PASRR evaluations.
This position includes maintaining a caseload, completing assessments and treatment planning on adults who have had a Level II PASRR Evaluation and been identified as in need of specialized services or other mental health services upon review. The evaluation is part of a multidisciplinary assessment which includes a diagnostic formulation, disposition and treatment recommendations.
Responsibilities:
- Monitors interventions by staff to ensure compliance with treatment plan objectives.
- Orients and trains new staff.
- Reviews documentation and signs off on assessments as needed.
- Approves/disapproves leave request.
- Monitors direct and indirect service delivery of OBRA staff.
- Assist in the development of new programs within the OBRA department.
- Provides on-going staff training and development.
- Ensures that all staff provides services that adhere to federal, state, and local guidelines for OBRA Pre-admission Screens, Annual Resident Reviews and Specialized Mental Health Services.
- Participates in employment interviews, performance evaluations and disciplinary actions as directed by Supervisor.
- Assessment skills and activity reflect an ability to gather data pertinent to consumer functioning and history.
- Provisional diagnosis formulation, disposition and treatment recommendation are consistent with the DSM-V.
- Communication skills reflect the ability to maintain positive professional relationships with other professionals within and outside of NSO-OAS.
- Provides clinical supervision with staff in the resolution of crisis situations presented by a client/family
- or nursing home to ensure that they are resolved in the least restrictive setting which is appropriate to the client’s situation.
- Provides clinical services to consumers on an individual or family support basis as required.
- Ensures timely and appropriate documentation consistent with services rendered.
- Maintain positive, professional relationships with external organizations.
- Remain available for consultation/training to nursing homes.
- Document all client and collateral contacts in compliance with agency policies.
- Attends and provides meaningful information at I-Team or BTC meetings in the development of treatment plans.
- Monitors and follows-through with treatment plan goals and objectives, as needed.
- Ensures timely and appropriate documentation consistent with crisis services rendered.
- Participates in clinical case conferences and case consultations as needed.
- Reviews staff documentation to ensure that it is accurate, appropriate, and submitted within required time frames.
- Documents every client and collateral contact in compliance with agency policy.
- Obtains accurate data collection for billing, fee-setting, the management information system and other purposes in accordance with agency policy.
- Completes all assigned reports.
- Completes monthly statistical reports of direct and indirect service hours.
- Completes internal billing reports.
- Performs other duties as required by supervisor and related to the position.

- Excellent writing, reading, and organizational skills,

Candidate Requirements:
- Master of Social Work from an Accredited University
- Licensed Master Social Worker (LMSW), through State of Michigan
- Minimum 2 years supervisory experience
- Minimum 5 years of clinical experience with the Severe and Persistent Mental Illness & Intellectual and Developmentally Delayed populations
- Experience working with nursing home personnel and geriatric populations (Preferred)
