Full-timeDescriptionAs a Non-Medical Case Manager you will provide various levels of case management assistance to individuals living with HIV/AIDS, many of whom seek help with core medical services, housing instability, food insecurity, and other issues.As a Non-Medical Case Manager, you will support the Southwest Center mission of providing affirming and inclusive services to promote well-being and advance health equity for diverse communities and all those seeking compassionate care - especially people of color, LGBTQIA2S+ and Queer individuals, and those affected by HIV.
What your day will look like at Southwest Center:
- You will conduct an initial assessment of services needed.
- You will complete individual service plans with clients to set goals and strategies to overcome barriers (including but not limited to mental health, chemical health, housing instability or criminal history) to maintaining medical care.
- You will have timely and coordinated access to medically appropriate levels of health and supportive services and continuity of care.
- You will continuously monitor and assess the client's efficacy of their care plan. Re-evaluation of the care plan every six months with adaptations as necessary.
- You will maintain client records and files, collect required documentation from clients, complete data entry of client notes, process information for program reports and invoicing, etc.in a prompt manner.
- You will participate in team wide activities and initiatives including trainings and meetings.

Requirements
Who we're looking for:
- You've earned your high school diploma, OR minimum of 2 years of case management experience.
- You have knowledge of social work and case management guidelines and issues related to working with low-income clients, including data privacy, confidentiality, and HIPAA compliance.
- You have knowledge of issues and community resources related to HIV/AIDS, chemical dependency, mental health, housing, health insurance, and other barriers.
- You have the ability to listen well and assist clients in de-escalation and problem solving.
- You have experience working with communities of color and or working with Gay/Bi/MSM/Queer and Transgender health issues. You must understand the complex issues our marginalized populations are faced with and the barriers they experience accessing healthcare.
- You have knowledge of and skills using personal computer equipment and related software for word processing (MS Word), spreadsheets (MS Excel) and CareWare.
- You have demonstrated experience preparing client files, records, and program reports.
- You have strong and timely communication skills with both clients and other professionals.
- You have the ability to work with clients respectfully and act in a confidential manner.
- You have the ability to manage, prioritize, categorize, and complete multiple tasks.
- You have the ability to manage stress in a proactive manner.
- You have demonstrated cultural competency to serve the LGBTQIAS+ communities professionally and respectfully.

- You have earned a Bachelor’s degree in social work or related field.
- You have personal or professional experience regarding people living with HIV/AIDS.
- You are Bilingual (Spanish English).
- You have experience with eCW.
Environmental Factors, Physical Requirements, and Compliance:
- Work primarily in a climate-controlled environment with minimal safety/health hazard potential.
- Office environment, exposure to computer screens for lengthy periods of time.
- The position requires extensive use of computers using a standard computer keyboard.
- This position may require evening and occasional weekend work.
- While performing the duties of this job, the employee is regularly required to walk, sit, stand; use hands, talk, and hear. The employee is occasionally required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus.
- Currently have, or be able to obtain within 90 days of employment, a valid Fingerprint Clearance Card
- Currently have, or are able to obtain within 7 days of employment, a clear TB test
- Currently have, or are able to obtain within 7 days of employment, a current flu vaccination
- Currently have, or are able to initiate within 7 days of employment, a Hepatitis-B primary vaccination series
- Currently have, or are able to obtain within 14 days of employment, a CPR certification
