Licensed Practical Nurse (LPN) - Mobile Caregiver

We are looking for a Licensed Practical Nurse (LPN) to expand our ability to provide care by literally meeting patients where they are.
They will facilitate convenient and empathetic blood and specimen collection and medication administration in patient’s homes, offices and other remote sites – always being mindful of each patient’s unique needs and comfort. This service-minded patient care professional will ensure accurate and efficient collection while maintaining the highest standards of care, confidentiality and accuracy in documentation.
What you'll do in the role:
- Make daily home/office visits, always maintaining a flexible schedule to accommodate patient needs.
- Greet patients warmly, establish rapport, and address any questions/concerns they may have regarding the blood draw process.
- Conduct in-home venipuncture and capillary blood draws for patients, adhering to strict infection control guidelines, ensuring a safe and comfortable experience.
- Secure blood samples and properly label, package, and transport them to our designated lab partner following established protocols.
- Administer patient medications as allowed per your nursing license’s respective scope(s) of practice in the state(s) in which you are licensed.
- Assist patients in setting up technology for telehealth visits, ensuring a seamless virtual healthcare experience with their respective care teams.
- Conduct other medical intake processes, including measuring blood pressure and gathering relevant health information.
- Complete all required paperwork and electronic documentation in a timely, accurate and compliant manner.
- Maintain a safe and reliable personal vehicle for transportation to and from patient sites, as well as a clean and organized mobile phlebotomy unit, ensuring all necessary supplies are safely stocked and ready for use.
- Uphold the highest standards of professionalism, confidentiality, patient care, and empathetic communication while always adhering to all regulatory requirements, including HIPAA and CLIA regulations.
- Other responsibilities as necessary to meet evolving business needs.
Education & Licensure Requirements:
- Licensure: Active, unrestricted license to practice as a Licensed Practical Nurse (LPN) in the State of Ohio required; Dual licensure in Ohio and Indiana or multi-state licensure preferred.
- Driver’s License: Valid state-issued driver’s license from the state of your residence with no limitations.
- Education: Undergraduate degree from an accredited college/university preferred.
Other Skills or Qualifications:
- Previous experience working in a caregiving/clinical capacity with patients in a home-based setting.
- Proven ability to quickly establish rapport and professionally interact with patients from diverse backgrounds in various domestic settings; some experience in hospitality/service industries preferred.
- Safe and reliable transportation capable of commuting to and from patient sites within a ~150-mile radius daily (mileage expenses paid by AndHealth).
- Willingness to adapt to changing technologies and healthcare practices.
- Ability to work independently and as part of a collaborative healthcare team.
- Strong administrative/technical skills; Comfort working on tablets/smart devices, as well as macOS and Windows-based computers using Microsoft Office (Outlook, Word, Excel, PowerPoint), Google Workspace (Gmail, Google Docs/Sheets/Slides, etc.) IM/video conferencing (Slack, Teams, and Zoom), and telephones efficiently.

- (PPD) TB Skin Test - Proof of negative TB skin test within the last 12 months.
- (MMR) Measles, Mumps and Rubella or A Blood Titer proving immunity.
- Varicella - (2) documented doses or A Blood Titer proving.
- Hepatitis B3 Series.
- Influenza (flu) required.
- COVID-19 Vaccine required.
