Summary: 


The Lead Medical Respite Coordinator oversees the Medical Respite Coordinators and Medical Respite Navigators, ensuring effective care coordination, client support, and collaboration with interdisciplinary teams. This role also carries a caseload and plays a crucial part in helping clients navigate the healthcare system, addressing barriers to permanent housing, and achieving positive client outcomes.  The on-site schedule is Monday - Friday, 8:30am - 5pm.  

Responsibilities: 


  • Coordinate all medical and social services following clients' discharge from the hospital and the medical respite site.
  • Assist with coordination of onsite telehealth appointments for mental and physical health services.
  • Coordinate all medication deliveries from local hospitals and pharmacies.
  • Identify and refer clients to appropriate local community resources.
  • Provide coordination of food services provided to patients.
  • Assist with patients' transition to their next housing location.
  • Participate actively in daily interdisciplinary care team meetings.
  • Communicate respectfully and professionally with all members of the care team to provide patient-centered care.
  • Act as a patient advocate and liaison between patients/families and community service agencies.
  • Support organizational initiatives to promote and maintain a strong positive workplace culture.
  • Always respect patient confidentiality and treat patients with courtesy and respect.
  • Perform other duties as assigned by supervisor, contractual obligations, or program description.
  • Provide leadership and supervision to the Medical Respite Coordinator and Medical Respite Navigator.
  • Conduct regular performance evaluations and provide constructive feedback.
  • Facilitate training and professional development opportunities for staff.
  • Ensure compliance with organizational policies, procedures, and regulatory requirements.
  • Foster a collaborative and supportive team environment.
  • Manage day-to-day operations of the Medical Respite Program, including scheduling and staffing.
  • Collaborate with other supervisors and departments to optimize program outcomes and resource utilization.
  • Serve as a resource and escalation point for staff and client-related issues.

Candidate Requirements:


  • Bachelor’s degree in social work, Psychology, Sociology, Public Health, or a related field Or Licensed Practical Nurse Or 3 years of experience in case management, mental health, or a related field
  • Limited License (If Applicable)
  • Full License (Preferred)
  • At least 2 years’ experience providing case management and/or care coordination services
  • Experience working with underserved population
  • Experience with identifying, accessing and referring to local community resources
  • At least 1 year, Supervisor experience (preferred)
