Summary:
The NSO Manager of Health Services oversees the day-to-day operations of our Medical Respite Program, located within our Detroit Healthy Housing Center. The Medical Respite Program allows individuals who are experiencing homelessness a safe place to rest and recuperate from a medical condition. While participants focus on improving their health, they can also work on addressing other needs Social Determinants of Health, with a targeted focus on housing. The Manager of Health Services is accountable for overseeing, managing, and mentoring the medical respite staff, to ensure patient centered care coordination is achieved, and client and program outcomes are maximized.
This position oversees our Medical Respite Coordinator Team and also provides nursing supervision.
Responsibilities:
Resident Care:
- Completes Intake Assessment upon initial admission to the medical respite site, to develop care plan and guide coordination efforts during clients admission
- Ensures there is ongoing development of comprehensive care plans tailored to the client short- and long-term healthcare and social determinants of health
- Ensures the care plans are monitored and evaluated for effectiveness and changing care needs and ensures appropriate action is taken in a timely manner
- Provide residents with a safe and comfortable environment
- Participates in client care conferences ensuring the care plan is being followed and exploring coordination of services with other NSO programs.
- Initiates unscheduled conferences to address concerns or to ensure awareness of the potential for additional or modified services appropriate to the clients changing needs.
- Ensure schedules and work assignments meet client and program needs
Leadership:
- Provides leadership and direction to medical respite staff by fostering and promoting new initiatives, interpreting and implementing policies and procedures
- Hire, orientate, evaluate, and educate staff; Directs, assigns and evaluates the work of medical respite staff
- Coordinates and communicates with other departments and staff to ensure clients needs are met
- Provide staff with a safe and comfortable environment
- Share information and participate on multi-site committees to develop and implement best practices throughout the organization.
Budget & Financial:
- Ensures that care programs are delivered in a professional manner: within the current budget allocation; and in accordance with provincial standards, legal requirements, and NSO policies and procedures
- Reconcile medical respite activity reports to ensure compliance with authorized days and levels of care & assess need for extended authorizations
- Is responsible for forecasting budget requirements and monitoring and approving expenditures including identifying and recommending new sources for products and services and providing rationales for budget variances
Occupancy:
- Responsible to review all admission requests for site appropriateness and provide final disposition for admission
- Works closely with healthcare facilities to facilitate appropriate and timely transfer of clients from their current accommodation to the medical respite site
- Ensures and coordinates discharges and transfers that are appropriate for the clients needs
Regulatory Compliance:
- Ensures that initiatives are based on audit/outcomes and indicators
- Participates in Continuous Quality Improvement through the completion of audits, observation and evaluation, ensuring compliance with NSO Standards, policies, and procedures.
- Participates in the development of polices and procedure for the medical respite site
Operational and Administrative Systems:
- Implements, coordinates, assesses and evaluates all activities of the medical respite site and program.
- Plans, organizes, directs and supervises all activities of the medical respite site and program to ensure the highest quality of service to the clients
- Ensures the maintenance of accurate and complete records of medical respite staff
- Develops and implements an inventory control system ensuring adequate supplies and equipment are available at all times within budgetary constraints.
- As an integral member of the site management team, participates in decision-making - particularly in matters related to health and safety, client care and care coordination, financial, and human resources management
Candidate Requirements:
- RN from and accredited university w/licensure through state of Michigan
- Minimum 3-years of Nursing experience in the LTC or Hospital sector
- Minimum 1-year of Management experience in the LTC or Hospital sector
- RN Case Management experience preferred vs minimum 1 yr of case management experience required
- Proficiency in MS Office (Word, Excel, PowerPoint & Outlook)
- Ability to learn additional software
- Proficiency in general office equipment (PC, printer/fax/copier, telephone system)
- Experience with electronic health records and scheduling systems
- Must have access to reliable private transportation
