Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children’s and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.
Job Description
The Lead Medical Coordinator is responsible for insuring the discharge and oversight of medical care for homeless clients from hospitals or other agencies contracted with Recuperative Care Center are being properly completed to the standards of Illumination Foundation Managers/ Supervisors/ Directors/ Board of Directors. The Lead Medical Coordinator will have oversight of facility and staff when Manager or Supervisor is not present. Lead Medical Coordinator will train and oversee other Recuperative employee’s and Interns within limits set by Supervisor, Manager and Directors.The pay range for this position is $30.00 - $34.00 per hour, depending on experience.
This position also qualifies for a $1.50 swing shift differential and a $3.00 graveyard shift differential.
Responsibilities
Lead Duties:
- Serve as a back-up for medical team when needed by covering shifts due to staffing shortages/site needs.
- Ensure completion of client documentation in KIPU and any external platforms in an accurate and timely manner by the medical team members
- Assist medical team with maintaining client work flows to ensure clients needs are met.
- Attend meetings and relay pertinent information to medical team members.
- With Site Manager’s guidance, provide support to team members by listening to concerns/feedback and resolving conflict/issues.
Client Care:
- Perform initial intakes to ensure client fitness for recuperative care and evaluate for any outstanding medical needs
- Documenting medical and mental health status of client upon arrival, checking vitals, determining insurance status and access to primary care provider, scheduling appropriate follow up appointments as outlined in discharge paperwork
- Working collaboratively with case management to link clients to medical services (insurance, PCP, specialists, referrals).
- Coordinate home health visits if needed
- Assist in promoting health literacy by providing patient education on specific disease processes, medications, preventative measures, treatment management and overall general health
- Arrange transportation for clients to and from medical appointments
- Report emergency situations to Supervisor/Manager and document incident
- Seek proper level of care to assess and treat medical complication
- Initiate medical response system when appropriate
Administrative/Record Keeping:
- Enter client information and case notes into Illumination Foundation’s EHR and other documentation systems.
- Perform administrative tasks such as answering phones and filing paperwork
- Maintain proper inventory of medical supplies and medications
Compliance/Safety:
- Maintain confidentiality in accordance with HIPAA guidelines and DHS standards.
- Provide clients with proper compliance forms upon admission to maintain client compliance with program rules
- Provide care utilizing infection control measures that protect both the staff and the client according to OSHA standards.
- Conduct medication reconciliation and ensure proper disposal and pickup of bio-medical/pharmaceutical waste.
- Maintain a safe work environment by ensuring the cleanliness of the med room, wound care room, and med storage room.
- Ensure medical equipment remains in operating conditions.
Preferred Experience/Minimum Qualifications
Required:
- High School Diploma or equivalent.
- Current and non-restricted LVN license in California
- 3 years of relevant experience.
- Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
- Valid CA Driver’s License.
Preferred:
- Bilingual in English and Spanish.
- Associates degree preferred
- Supervisory experience
- Proficiency in Microsoft Office Suite (Word, Excel, Outlook, Teams)
- Home Health nursing experience and experience working with populations dealing with addiction and mental health conditions.
