Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children’s and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description


The Associate Manager of Case Management is responsible for overseeing all CalAIM recuperative care and short term post hospitalization related processes and assisting the manager of case management with the supervision of Los Angeles case management staff. The associate manager is responsible for attending all health plan ICT's, ensuring all CalAIM contract deliverables are met, and reviewing and submitting authorization requests. The associate manager will train and supervise case management leads and ensure they are properly onboarding and supervising case management staff.

The associate manager is expected to assist with service provision for high acuity clients and developing external relationships with other service providers. The pay range for this position is $29.00 - $30.00 per hour, depending on experience. 

Responsibilities


Supervision

  • Supervise case management staff for daily tasks, case oversight, training, and growth.
  • Coordinate hiring process of new case management staff.
  • Work with Site Managers on all client decisions including exits, extensions, intakes, and other case planning action steps.
  • Coordinates orientation and ongoing in-service education for staff to improve skills and client care

Client Care

  • Provide case consultation and emergency management for case oversight.
  • Provide management support to assist case managers and clients in creating crisis management plans or plans to support client’s progress towards self-sufficiency. Plans can include interventions for sobriety, safety, self-harm contracts, employment, housing retention, increase in income, and child safety.
  • Oversee care coordination in partnership with Program Managers and Site Managers for all programs to ensure client care, effective case planning, and positive outcomes in line with the mission of the organization and contract outcomes.

Documentation

  • Ensure accuracy of data for contract compliance, grant reports and other reporting needs.
  • Assists Program Manager with contract compliance as requested.

Collaboration

  • Attend community meetings and collaborations to build relationships and partnerships.
  • Coordinate resource presentations for case managers to increase knowledge of community resources.

Preferred Experience/Minimum Qualifications


Required:

  • Bachelor’s degree from an accredited college or university in Social Services or related field.
  • Minimum 2 years of recent progressively responsible administrative management/supervisory experience.
  • Previous experience working as Case Manager or similar position
  • Experience in supervision or project management and oversight.
  • Current valid CA driver's license
  • Have a dependable vehicle with automobile insurance
  • Computer literate; ability to use Microsoft Office products and databases such asHomelessness Management Information Systems (HMIS)

Preferred:

  • Masters Degree in Social Services and 2 years experience in the Social Services field.
  • At least 2 years of experience working with at risk/homeless individuals
  • Bilingual in Spanish and English is helpful, highly desirable
