Job Title: Community Health Nurse - High Risk Perinatal Program


Southwest Human Development is seeking a Community Health Nurse for the High Risk Perinatal Newborn Intensive Care Program (HRPP/NICP). The HRPP/NICP provides in-home community health nursing services to assist families and their newborns during the transition from the Newborn Intensive Care Unit to their home and community. This position is for a contract nurse to do 15 visits a month, paid per visit.As a Community Health Nurse, you will work in collaboration with families and other community providers, complete developmental screenings, provide support and education on development, infant and home safety and health prevention/promotion.

You will attend regular team meetings, case conferences and have regularly scheduled reflective supervision time to support you in your work.

In this role, you will:


  • Identify high-risk infants and children through hospital (Levels II and Level III) and community referrals from Maricopa County.

  • Participate in/facilitates the exchange of information from hospital to nurse to program family.

  • Facilitate exchange of referrals to appropriate nurse.

  • Explain the benefits and responsibilities of the follow-up program to families in a manner which is sensitive and responsive to individual family needs.

  • Work in collaboration with families to assess their educational, social, health and economic needs, develop an intervention plan and assist them in attaining their established goals.

  • Provide on-going assessments using appropriate assessment tools, observation techniques and listening skills.

  • Promote family self-sufficiency related to health issues, and measures desired participant and family outcomes.

  • Provide verbal and written information to families to promote/support

o Increased understanding of growth and development norms.o Increased knowledge of health promotion practices (including prevention).o Importance of parent-child interaction that is age and developmentally appropriate.

  • Identify children with physical and/or developmental problems and provides appropriate referrals for evaluation and intervention.

  • Identify resource inadequacies, make appropriate referrals for service and follows up to ensure that services are received.

  • Develop and maintain a professional, non-judgmental, mutually respectful relationship that supports confidentiality and respects the privacy of individual families.
