Physical Therapist (Home Health) – Albany County

What You'll Do:
- Assess patients' physical and functional needs at home.
- Develop, implement, and modify individualized treatment plans.
- Educate and guide patients, families, and home health aides.
- Recommend and assist in obtaining necessary assistive devices.
- Coordinate care with PTAs, HHAs, and other healthcare professionals.
- Maintain accurate and timely documentation of patient progress.
- Follow infection control protocols and uphold patient confidentiality.
- Participate in team meetings and performance improvement activities.
What You Need:
- NY State PT License (required)
- 1+ year of physical therapy experience (geriatrics preferred)
- Home care experience (preferred)
- Ability to work independently and travel within Albany County
