What You Will Do
- Manage cardiovascular patients via telehealth, delivering expert care remotely.
- Work closely with cardiologists as a key member of their virtual practice.
- Assess, diagnose, and monitor patients with low-acuity cardiovascular conditions.
- Ensure seamless coordination of care between telehealth and in-person cardiology teams.
- Educate patients on risk factors, medication adherence, and lifestyle changes.
- Utilize telehealth platforms and EMRs to enhance efficiency and patient outcomes.
Who We Are Looking For
- Certified Nurse Practitioner (NP) licensed in Maryland. Licenses in D.C. and Virginia preferred but not required.
- Three or more years of experience in cardiology, preferably in an outpatient clinic setting.
- Medicare and Medicaid enrollment, DEA number required.
- Strong familiarity with telehealth platforms and remote patient monitoring.
- Ability to work independently while collaborating with cardiologists and clinical teams.
