Summary: 


This position promotes the mission, vision, values and strategic plan of Neighborhood Service Organization and helps to achieve the strategic goals and objectives of the unit. This is a clinical position providing outreach mental health assessments pursuant to PASRR regulations. Assessments are conducted on adults who have been recommended for a Level II Pre-admission Screening or Resident Review. The evaluation is part of a multidisciplinary assessment which includes a diagnostic formulation, disposition and treatment recommendations.

Responsibilities: 


  • Responsiveness to referrals reflects a thorough understanding of the Guidelines for determining when it is necessary to complete a Level II Assessment.
  • The quality of the Level II Assessment documentation reflects consistency in familiarity with the Department of Community Health (DCH) PASRR Operation Manual Guidelines for completing a comprehensive Level II evaluation.
  • Assessment skills and activity reflect an ability to gather data pertinent to consumer functioning and history.
  • Provisional diagnosis formulation, disposition and treatment recommendation are consistent with the database.
  • Communication skills reflect the ability to maintain positive professional relationships with other professionals within and outside of NSO-OAS. II. PRODUCTIVITY
  •  Responds to Level II referral assignments within contract guidelines as evidenced by the timely completion of a minimum of 1 1/2 Level II evaluations per working day. (PAS= 2, RR = 1, and Face to Face Partial with Documentation = 1/2). * Consideration will be given to Evaluators who have additional clinical assignments added to their primary responsibility of Level II Assessment Screening productivity.
  •  Level II Assessment documentation as submitted is consistently accepted upon initial review by the PASRR Supervisor for processing without a request for resubmission relative to incomplete detail, typing errors and/or unacceptable accuracy.
  • Level II Assessment documentation upon submission consistently reflects knowledge of the information sought in each specific area of the Level II assessment as specified in the DCH PASRR Guidelines for completion of a comprehensive Level II Evaluation as evidenced by consistent acceptance upon Supervisory review.
  •  Diagnostic Formulation in the Level II Assessment documentation is based on the DSM criteria.
  •  Consistently responds to clinical matters as directed by Supervisors within the expressed promptness of the request.
  • Consistently attends and participates in a constructive manner at clinical meetings as scheduled by Supervisors.
  • Consistently complies with NSO-OAS training requirements and selects training opportunities each year to meet the 24 hours required in this clinical position.
  •  Consistently initiates the scheduling of monthly one- on- one supervision to review productivity and attends the scheduled supervisory session promptly, and comes prepared with documentation of data reflecting the previous month's performance for review.

Candidate Requirements:


  • Education: M.A. (Psychology or Counseling); or MSW. 
  • Required Certification/License: 

  • Licensed Master Social Worker (LMSW)
  • Limited Licensed Master Social Worker (LLMSW)
  • Limited Licensed Psychologist (LLP) 
  • Fully Licensed Psychologist (LP) 
  • Licensed Professional Counselor (LPC) 

    • Additional Skills: Course work or experience with older adults or individuals with serious and persistent mental illness and basic computer skills. 
    • Requirements: Valid Michigan driver’s license and proof of auto insurance
