Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children’s and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

Job Description: 


The Recuperative Care Lead Care Manager (RC LCM) is a site based, client-facing, Care Management role wherein populations of focus are sought out, engaged, assessed, enrolled, and advocated for on a routine basis. This person is the main point of contact for clients intermittently housed with Illumination Foundation. RC LCMs build strong relationships with clients to help them stay engaged in their medical care, behavioral health services, and social support. This is a holistic non-clinical role requiring adherence to and application of evidence-based practices, knowledge of client and service barriers as well as social determinates of health, and providence of appropriate coordination of services to populations of focus.

They help navigate health care services and systems, promote health and preventative care, and work closely with the client’s Care Team. This roles pay range is

$22-$25.00 per hour. 


Job Duties:


Client Needs:


  • Ensure comprehensive case management is provided by: Evaluating client’s need, developing treatment plans, monitoring progress, aiding clients, referrals, following up with weekly goals and completion of action steps 
  • Complete care plans and utilize Electronic Health Record(s) (EHR) and client databases (i.e., Champ, HMIS, Health plan programs) to track and maintain accurate case notes, client, and program data in SMART Format where applicable.
  • Collaborate with the Case Management Team by attending weekly meetings to discuss the effectiveness of programs and client progress, and create new strategies to meet client’s level of need and improve treatment plans 
  • Link clients to resources to assist patients’ psycho-social and daily needs such as health care, nutritional needs, hygiene items, and referrals to other agencies for transitional/permanent supportive housing, and care team members including but not limited to: Care Coordinators, primary care physicians, and other health care providers
  • Perform crisis intervention, as necessary 
  • Establish and maintain confidential case files for participants and review required statistical reports for program management and evaluation. 
  • Maintain communication with related outside agencies 
  • Promote monthly Health Promotion topics/materials.
  • Accompany clients to medical appointments as needed and/or coordinate transportation as needed.
  • Carries a caseload of a maximum of 30 ECM members, unless instructed otherwise by senior management within the scope of policy guidelines 
  • Prepares for and participates in individual and group supervision meetings
  • Provides daily End of Shift (EOS) reports to track performance measures 

Documentation


  • Responsible for documenting all engagement and frequency of content in Illumination Foundation’s EMR in accordance with standards of practice as required by Illumination Foundation and associate contract entities. These Practices include: 
  • Incrementally documenting all aspects of a client care plan, including goals met as well as goals forthcoming
  • Documenting level of engagement such as frequency and duration of encounters
  • Documenting evaluative client case details which factor into decisions made to referral clients to alternative resources
  • Documenting obtained client records, such as vitals documents, insurance cards, SSI award letters, etc.
  • Documenting client disengagement as well as engagement
  • Remain aware of current services offered by other services providers
  • Maintain strict confidentiality in accordance with agency policies
  • Track client information, schedule, files, and forms
  • Track client attendance at medical appointments and patient navigation sessions and initiative outreach and missed appointment procedures, as necessary

Required:


  • Bachelor’s degree in social services, Healthcare, or related field; or equivalent combination of training and experience.
  • Experience in homeless services, case management, and mental health support
  • Current valid CA driver's license
  • Experience in non-profit housing and/or housing for people with disabilities and chronic health conditions.
  • At least 1 year of experience working with at risk/unhoused individuals (preferred)
  • Must be familiar with VI-SPDAT.
  •  Bilingual in Spanish and English (preferred)
  • Knowledge of resources available in corresponding counties

Preferred:


  • Bilingual in English and Spanish.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Calendar, etc.)

Studies show that families with children are the fastest growing segment of the homeless population in America. This is especially true in Orange County, where one of the most expensive housing markets in the nation creates an immense gap between income and affordable housing.
