Based in Indiana, you’ll work out in the community—meeting patients in their homes, focusing on prevention, not just treatment. It's a mix of in-home visits and limited travel across the state (2–3 times a month max, with the occasional overnight stay—no more than 3 days at a time). You’ll help seniors stay healthy before chronic conditions take over.

What You Need


  • 1+ year as a Medical Assistant (CMA, RMA, or similar)
  • 6+ months in Internal/Family Medicine or another primary care setting
  • Experience doing Medicare Annual Wellness and Well-Child Visits
  • Comfortable going into patients’ homes (home health experience is a plus)
  • Based in Indiana and open to light in-state travel (2–3 short trips/month with some overnight stays)
